Maybe we all feel collectively uncomfortable that we recognise that she is conventionally unattractive and the unfairness of life which leads to feeling cognitive dissonance, and end up expressing it as concern for her or outrage that she’s being used in a meme? When the crux of the joke may not really be her appearance, but rather the premise, someone peering into a box with a saccharine smile, photographed from an awkward angle inside the box itself.
I think the meme is funny regardless of who is portrayed as the ‘titan’.

And really, if it was a conventionally handsome man, it would likely be more “making fun of his appearance” than it is with this lady. A dude with a chiseled jawline looks way more like a titan physically, and therefore his looks would be more part of the joke.
